Dear Secure@Home Members,

Over the past few weeks it has come to our attention there is some confusion about the full scope of Secure@Home’s services and capabilities. This specifically has to do with members who become ill, injured and/or hospitalized. Over the next few weeks, we will be sending out more detailed information, but I wanted to alleviate the confusion immediately by reminding you of our wide range of services. Please read this letter carefully.

As you know, Secure@Home is here to help you remain in your own home. Our services include: 24-hour emergency phone assistance, cultural and wellness programs, Chore Corps to help with household tasks, Preferred Provider service when you’re looking for a vendor, health or home service, Monthly Hellos and transportation discounts. These services are designed to help you maintain your lifestyle and with your input, we continue to build on our program benefits every day.

However, we are also Care Managers and your Secure@Home membership includes Care Management. We have licensed clinical social workers on-staff to provide comprehensive coordination of all of your health-related services.

If you are ill, injured or hospitalized, please call us immediately. We can:

Help to provide day-to-day necessities such as food and medicine, guidance and information regarding medical problems.

Accompany you to a doctor’s office.

Act as your advocate and help you and your loved ones when considering options and making decisions about your needs.

Work with your physicians to ensure that you are receiving the best care possible (provided you’ve signed our HIPPA release).

Help you address insurance issues including Medicare and long term care policies.

Help maintain contact with loved ones and friends and update them on your progress.

Provide the hospital with any relevant medical and contact information that has been collected from your assessment and care plan.

Work with discharge planners at the hospital to arrange post-hospital care.

Work with home health agencies to ensure you get the aides you need (some of whom provide discounts to S@H members).

Work with companies to help arrange for rehabilitation equipment or any type of therapy.

Bring personal items to the hospital that you need from your home.

Make sure that your home is being cared for while you are in the hospital such as picking up your mail and newspaper.

Help you determine if you home needs temporary or permanent modifications.

Ensure you have the resources arranged to return home such as groceries and medications.

Care Management is part of your Secure@Home membership fee and covers 3 hours per individual and 5 hours per couple on an annual basis. After your initial hours are used, the fee is $75/hour. These care management hours will be billed in 15-minute increments with the first five minutes at no charge. After 20 hours/25 hours of paid care management are used within one year, the rate will be $50/hour. Travel time beyond 15-minutes (one way) from the Secure@Home office will be billed at $40/hour.

Please keep in mind that Care Management does not apply each time you contact us for general information, Chore Corps, Preferred Provider information, Monthly Hellos, transportation, etc. These services are included in your membership. As a comparison, most private geriatric care managers charge per contact with members regardless of the type of question/involvement.
